How to Start Hospice Services
If you would like to explore if hospice care is right for you or your loved one, please call us at 800-789-0586 to speak with an admissions consultant. If they happen to be talking with another patient at that time, our office staff can answer most general questions. Within an hour, you can usually speak directly with an admissions consultant, who will set up a meeting to review the benefits of hospice, the services available and how they are covered. There’s a great deal of information throughout our website as well.
It is important to remember — your hospice provider is your choice and you can refer yourself to hospice. If you would like same day admission, Gentle Shepherd can also make that happen!
So, what is the process?
- To self-refer, you can call us at 800-789-0565 or complete this online inquiry form — or you can let your primary care physician know that you have decided to engage Gentle Shepherd Hospice. Either way, your doctor will need to be notified so their office can send us the medical paperwork necessary for admission. This includes: your history and physical background, recent progress notes, demographic sheet and MD order for hospice.
- Once we receive the paperwork, we can usually review it right away (or within an hour) to confirm that you meet the hospice criteria for your diagnosis and qualify for the coverage benefit. Sometime a clinical evaluation by a registered nurse might be scheduled to ensure that you are appropriate for hospice.
- Immediately upon admission, your personalized hospice care plan will be developed. Any medical equipment and supplies pertaining to your terminal diagnosis that you need will be ordered and sent to where you live.
- You will be immediately added to our care team schedule and can expect your first visit very soon.
